* Do you have a GPA of 2.5 or higher?

NOTE: The Baccalaureate Degree Health Information Management Program is in Candidacy Status, pending accreditation review by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM).

PLEASE READ AND INITIAL EACH OF THE FOLLOWING STATEMENTS:

* A background check and drug screening are required after acceptance to the Health Information Management Program and prior to clinical assignments. If you have problems on the background check, this may prevent you from going to some or all of the clinical sites. If you cannot go to any clinical site, you will be dismissed from the HIM program.

* This application is only good for the current application term. If you are not accepted and wish to reapply for the next cohort, you MUST submit another application packet.

* ALL Information and documents requested MUST be received WITH this applications. This includes official transcripts from ALL other Universities, Colleges, or Technical Schools you have attended.

* The admissions process is tedious and lengthy. Applicants will be notified by U.S. Mail and/or your Gordon student email to the address listed on your Student Academic Summary when decisions are made. MAKE sure your address is correct in your Banner Web Account.
